National Association of Chronic Disease Directors
The National Association of Chronic Disease Directors (NACDD) makes it possible for state and territorial health departments to work more effectively to prevent chronic diseases by providing advocacy, funding, and support in communities across the country. Our task was to highlight the successes of a pilot program designed to create inclusive healthy communities nationwide.
We captured four impactful stories from rural, underserved communities in Idaho and Montana. These were used to celebrate the program and promoted locally within each community to increase awareness and show how when everybody matters, everybody wins.
